Who may be eligible
- Ages 0–20 at the time of enrollment
- Eligible for Ohio Medicaid (managed care or fee-for-service)
- Shows behavioral health need on a CANS assessment
- May qualify after a behavioral health-related hospital stay

in primary flex funds many enrolled families can access for the practical needs their care plan calls for.
Starting with a CANS assessment
The Ohio Child and Adolescent Needs and Strengths (CANS) assessment looks at a young person’s strengths and needs — and their caregivers’ — to determine eligibility and shape the care plan. You can request one through your managed care organization, Aetna Better Health of Ohio, a local care management entity, the Medicaid Consumer Hotline, your behavioral health provider, or your county Family and Children First Council.
Once eligibility is confirmed and the CANS is entered, enrollment happens right away and a care coordinator reaches out within a few days.
